Questionnaire Functionality in Workplace Service Catalog
Carry over the functionality from Web central for the Service Desk Questionnaire functionality. Or ability for administrator to configure questions/data points on Service Catalog requests.

In Release V2023.01, this feature was released for service requests only. Extending to move requests, Maintenance Console, etc., is still under consideration

The main thing to remember is that all clients use this for different types of requests. So there could be a lot of questions with different types of responses (memo, pick list, select from a table, etc.) so capability needs to mirror current service desk questionnaire. Each questionnaire would have it's own set of questions, but that's not to exclude Fred's idea of placing in the SLA for the Service Request (cannot become a work request).

Maybe the questionnaire can be expanded to be used for Satisfaction Survey, refer to item below:

The workplace portal is well received for end users but it doesn't have the ability currently to define questions or otherwise that can be now storable as a UDF within the archibus framework. The clients are looking for methods where depending on the type of request, that they can have a certain questions defined. . For example one question/UDF may be:
Branch - this is not a department by the way
Customer address - given there will be a lot of non users
there are others I'm sure.After in the Service Console, they do hope they can search for some UDFs. ie. by customer address for example.

This is also important for the integration between modules, specifically Move Management and Building Operations.
Since most organizations who buy the license for Workplace will have their staff use this interface to submit move requests and Maintenance Requests, the workplace service questionnaire also needs to tie to the information that is visible on the Move Console and Building Operations Console (WR Details).
For example, the questionnaire information for a Maintenance Request needs to be added to the WR so that the craftsperson has the ability to look up this information from Web Central Maintenance Console and Mobile app.
Similarly the questionnaire information needs to propagate to the Move Request and be visible on the Move Console. -

This would be a good additional to the Workplace service catalog, especially as people come back to work and may need to fill out a daily health attestation.
User Story:
As a facility manager responsible for the health and wellbeing of staff coming back to the office, I'd like each employee to fill out a daily health attestation so that we can monitor who is coming back and what answers they gave.Implementation notes:
You could have a service request type of "HEALTH FORM" (or the like). We'd have to figure out the best way to attached the questionnaire to the request:
- By request type as in the older questionnaire functionality
- Somehow by SLA as in the newer questionnaire functionalityIf by request type, use the new questionnaires.
If by SLA, we'd do something like create an workflow step in the Requested status, but it would be 2-step process unless we have workflow (by request type for example) that auto-requests an then calls workflow steps.
